In response to COVID-19, the student health examination (hospital check-up included) is cancelled this semester.
★Notice of Health Examination:
Undergraduate junior students: All health examinations in the 2nd semester of 2019/2020 are cancelled. There is no need to submit a health record as well.
Transfer students (2rd & 3rd year): Please submit either the health record from previous school, a health record within a year, or do the health examination next semester.
Early admission graduate students: Please submit either a health record within a year, or do the health examination next semester.
*The release date of “a health record within a year” mentioned above has to be after 1 March 2019. (Military entrance medical examinations are acceptable.)
★Refund of Health Examination Fee:
If you have already paid and haven’t go to the hospital to do the examination, you can get a full refund.
Here are the refund options:
- Directly transfer to the bank account you’ve provided at your admission (“Chunghwa Post” or “Mega Bank”).
- Anyone who hasn’t provided a bank account back then should bring either their own passbook, the copy of their own passbook, or ATM card (contains the bank account information) of “Chunghwa Post” or “Mega Bank” to Cashier Division on the 1st floor of Dickson Lee Hall to make a bank transfer. The deadline is Monday, 23 March 2020 at 5 PM.
- If you didn’t provide your bank account in time, you can still get a cash refund:
(1) Time: from 25 March 2020 to 30 April 2020, Monday to Friday, 8:30 to 16:30.
(2)Procedure: Bring your Student ID card to Accounting Office on the 6th floor of Dickson Lee Hall to get a refund form, which would help you get the cash at Cashier Division on the 1st floor of the same building.
*During the epidemic period, avoid public places, manage your personal hygiene, wash your hands well and often, and maintain cough manner.
